We’re looking for an outstanding Support at Home Coordinator to join our team in the new year who will be able to utilise their existing knowledge to comprehensively assess our clients and implement support services as required.

About the Role

As the Support at Home Coordinator, you will coordinate the service delivery and be responsible for supporting the needs of our Aged Care clients subscribed to HenderCare's Home Care Packages. You will plan, implement, and manage all aspects of our Home Care Packages as advised by the client to ensure the service delivery meets their expectations and the expectations of relevant stakeholders. 

 

In this role, you can expect to:

  • conduct home care assessments
  • manage individual budgets
  • assist clients to navigate the My Aged Care services
  • provide effective, responsive, goal-focused and outcome-based service provision
  • provide advice and advocacy that supports the promotion of individual rights and choices for consumers and participants
  • Full case management required

 

With access to HenderCare fleet vehicles, you will be required to travel to NSW and the ACT to meet with clients.

 

About You

With your exceptional communication and interpersonal skills, you will liaise with a diverse range of participants, their families, service providers, and internal and external stakeholders to ensure your support meets expectations.

 

Your keen attention to detail will allow you to prioritise tasks and meet deadlines while consistently demonstrating a commitment to high-quality service delivery. You will harness your problem-solving skills, sound judgement, and initiative to effectively present recommendations for the client, based on their needs and advice.

 

You will be expected to have experience in:

  • Completion of a tertiary qualification in Aged Care or relevant field.
  • The administration of Consumer Directed Care, HACC, or Community Aged Care Packages to frail, aged, and/or disabled persons;
  • Coordination, monitoring, and reviewing service in a community setting, including formulating agreed Care Plans;
  • Familiarity with an Integrated Management System that operates in an environment of continuous improvement is desirable.

 

A sound knowledge of government and non-government health services is desirable. Qualifications as an Enrolled Nurse will be advantageous.
